Children under the age of seven years are welcomed into life in Christ and the Christian community through the sacrament of Baptism.  Families are encouraged to contact the parish office as soon as possible when expecting a child, or shortly after their birth to secure availability of your preferred date/time. You may call the parish office at 502-459-4251.

Baptism Registration Form must be completed and submitted prior to the scheduling of the sacrament.

  • Once this form is received, someone in the parish office will contact you to discuss the baptism plan.
  • Please return the completed form by email, or by dropping it off at the parish office.  
  • Parents must attend a preparation session before celebrating their child’s baptism. 

Choosing Godparents

Requirements to be a Catholic Godparent:

“Each child may have a godfather and a godmother, the word ‘godparents’ is used in the rite to describe both.”

Only one godparent is necessary. The godparent must be a fully initiated Catholic (Baptized, Confirmed, received Eucharist), at least 16 years old, and must be leading a sacramental life in harmony with the church. (Cf. CIC, can. 874) The godparent should be willing to accept the responsibility of assisting the parents in developing the faith life of the child.

The godparent may not be the father or the mother of the one to be baptized. (Cf. CIC, can. 874) Although people of some cultures sometimes choose multiple godparents, only two names can actually be entered in the Baptismal Register. (CIC, can. 873)

A baptized and believing Christian from a separated church or community may act as a Christian witness at the request of the parents, but there must be a Catholic godparent. (Cf. CIC, can. 874.)

Godparents should be invited and encouraged to attend Baptismal Preparation in order to better understand their role as godparents. If they are from another parish, a letter of recommendation from their pastor may be required.

Section Break

Each child to be baptized must have at least one godparent. The godparent must be at least 16 years of age and have fulfilled all three sacraments of initiation-Baptism, Confirmation and Eucharist. BAPTISMAL CERTIFICATES FROM THE PARISH OF RECORD, WITH NOTATIONS, FOR GODPARENTS MUST BE SENT TO ST. JOHN PAUL II PRIOR TO BAPTISM. Also, it is expected the godparent be an active Catholic in good standing. A second godparent, who meets the same qualifications as the first, is desirable. If there is a second godparent, each gender must be represented. At least one godparent must present at the time of the Baptism.

Section Break

It is necessary for parents to attend a Baptism preparation workshop prior to the Baptism. Schedule with the parish office. Both parents are highly encouraged to attend and are welcome to bring children with them, if needed. Godparents are encouraged to attend as well.
